The clinical problem
Dyspepsia volumes are high and endoscopy capacity is finite. Serology can't separate a past H. pylori exposure from an active infection; a rapid urease test can read falsely negative if the patient took a PPI within two weeks, or antibiotics or bismuth within three weeks, of the scope. And gastric-cancer risk is usually addressed only once symptoms force a scope.
